לטיפול ההורמונלי החלופי מספר מטרות, ביניהן מניעה של מחלת לב איסכמית בנשים בתקופה שלאחר חידלון וסת. הקשר שבין טיפול הורמוני חלופי למניעה של מחלת לב נבדק במספר מחקרים. אולם יעילותו של טיפול זה במניעת מחלת לב איסכמית בנשים לאחר חידלון-וסת, אינו ברור. מבחינת המניעה הראשונית מלמדים המחקרים השונים על תוצאות לא עקביות, וקיימים חילוקי דעות בין הטוענים להיעדר השפעה מונעת ראשונית לבין הטוענים, כי ניתוח תוצאות המחקרים השונים אינו מדויק, וכי לטיפול אכן יש השפעה המונעת מחלת לב איסכמית. במחקר HERS בנושא מניעה שניונית של מחלת לב איסכמית הודגמה דווקא עלייה בשכיחות האירועים בעקבות טיפול הורמוני, אם כי עלייה זו חלה בעיקר בשנה הראשונה לטיפול, ובהמשך הסתמנה ירידה בשכיחות אירועים אלה. הסיבות לעלייה בשכיחות האירועים מצד הלב אינן ברורות, אם כי אפשר שהדבר כרוך בהתגברות התהליכים הפקקתיים, הדלקתיים והאיסכמיים בתת-אוכלוסיות מסוימות של נשים עם מחלת לב איסכמית.

Exhaustion of Motor Cortex after Head Injury Revealed by Trans-Cranial Magnetic Stimulation

We evaluated the pattern of motor evoked potentials elicited by single-pulse and slow-rate (1 Hz) repetitive, transcranial, magnetic stimulation (RTMS) in minor head injuries. The motor response to a single magnetic stimulus in patients with minor head injury was characterized by a significantly higher threshold than in healthy subjects. However, central and peripheral motor conduction was normal in all patients. A stable pattern of MEP throughout the RTMS session was the most prominent feature in the control group. A progressive decrease in MEP amplitude and irregular alternation of large and very small MEPs over the course of RTMS was observed in minor head injury. The higher threshold of the motor response and the abnormal patterns of MEP behavior revealed by RTMS may reflect impaired excitability and enhanced exhaustion of the motor cortex in patients with minor head injuries, which improve with time.

Diskitis in Brucellosis not Responsive to Anti-Brucella Treatment

A 14-year-old girl with fever and low back pain was diagnosed as having brucellosis and diskitis. Treatment with doxycycline and gentamicin had no effect nor was there any improvement when the treatment was changed to doxycycline and rifampicin. With cessation of this treatment and administration of cloxacillin fever was reduced and the back pain disappeared. After 12 days of IV cloxacillin, she was released from hospital with instructions to continue cloxacillin orally for another 4 weeks. We conclude that when a patient is suffering from brucellosis and diskitis and does not respond to the usual anti-brucellosis treatment, an anti-staphylococcal preparation should be added.
