        ההתפרצות של קדחת מערב הנילוס (קמ"נ) (West-Nile Fever) בסוף קיץ-סתיו 2000 גרמה לבהלה ואף לחרדה בקרב הציבור, אך יחד עם זאת גם עניין רב וסקרנות מדעית במערכות הבריאות, איכות הסביבה והרפואה הווטרינרית, ומקום מרכזי בכל אמצעי התקשורת בישראל ובעולם. נראה היה שמדובר ב"מחלה אקזוטית" חדשה, כפי שמעיד עליה שמה – צירוף של מחלת "הקדחת" יחד עם "מערב הנילוס".

        בחוברת הנוכחית של "הרפואה", מופיע מאמרם של נאסר וחב', מבית החולים לגליל המערבי בנהריה, שבו מדווח על ההתמודדות של הרופאים במחלקה לרפואה פנימית עם האיבחון והטיפול בחולה קמ"נ הראשון בגליל המערבי, שלקה בתסמינים נירולוגיים קשים ונזקק לאישפוז ולטיפול נמרץ.

        לכאורה התוודע ציבור הרופאים בישראל לקמ"נ רק לאחרונה, ובעיקר לתסמיני המחלה הקשים, המצריכים אשפוז במחלקות לאשפוז נמרץ ואף עלולה לגרום לפטירת החולים. אולם רק מעטים יודעים, כי הפרסומים הראשונים בספרות העולמית על ההיבטים הקליניים של מחלה זו הגיעו בשעתו ממדינת ישראל, בשנות קיומה הראשונות.

        Eosinophilic Ascites - Presenting Symptom of the Hypereosinophilic Syndrome

        We report a case of a 22 year old man with a history of bronchial asthma, suffering from diarrhea, eosinophilic ascites and prominent blood and bone marrow eosinophilia. The patient responded to corticosteroid treatment.

        Later, hydroxyurea was added to this treatment because of recurrence of eosinophilia, ascites and hepatosplenomegaly.

        Eosinophilic ascites is rarely the outstanding symptom of the hypereosinophilic syndrome.

        Following the exclusion of other reasons for eosinophilia, concomitant unexplained hepatosplenomegaly suggested the diagnosis of a hyperereosinophilic syndrome.

        The Effect on Family Members of Treating Home-Hospitalized Patients

        Background: In recent years, chronic patients being treated at home are being joined by increasing numbers of acute and sub-acute patients. Although there has been a considerable expansion of formal services for home hospitalization, nevertheless a significant part of the burden of caring for these patients falls on their families. Many studies have examined the effect on the family of treating chronic patients at home, but very little is known about the effects on the family caregiver of treating an acute or sub-acute patient at home.

        Goals of the study: a. To determine the main areas of care involving family members within the framework of home hospitalization. b. To assess the caregivers' sense of burden. c.To identify the causes of the feelings of burden. d. To examine the caregivers' specific difficulties. e. To identify the areas in which they are particularly interested in additional guidance.

        Methodology: The study population included 179 primary caregivers of patients registered in four home hospitalization units. The data were collected during face-to-face interviews with the primary caregivers, in the patients' homes.

        Findings: Half (49%) of the caregivers were the patients' spouses, and the rest were other family members, 67% were women. The average age was 56, and about a third were over 65. On average, they were involved in six areas of care. Fifty three percent reported that the burden they had to contend with was too heavy, 28% said it was not so heavy, and 19% said it was not heavy at all. A multi-variate analysis revealed that the feeling that the burden was too heavy had a statistically significant correlation with caring for another family member at home, being involved in 3+ areas of treatments, being married, 9+ years of education, the patients suffering from frequent pain or unable to reach the bathroom independently. Most (75%) of the caregivers reported three or more areas of burden which include the time devoted to care giving, and the effect of the treatment on the patient's health, mental and physical condition. Although 77% of them had received instruction on how to treat the patients, 29% cited at least one area in which they would have liked to receive more instruction, noting in particular the need to learn how to deal with changes in the patient's mental status.

        Conclusions: Family members caring for home-hospitalized patients require more instruction and advice from the formal agencies during the daily care of the patient, particularly in specific subjects. There is a need to develop additional instruments for advice and instruction, in a broad range of areas, in order to assist the caregivers in their complex role. There is a need to continue to examine specific areas in which the family members require additional counselling and instruction, in order to better plan these services, and help improve the quality of care for home-hospitalized patients.

        Long-Term Sequelae of Malignant Tumors in Childhood

        110 children with malignant diseases (leukemia excepted) who survived 5-20 years (median 9) post-therapy were followed (1996-1998). Median age during follow-up was 15 years (range 5-23). The most common malignancies were brain tumors, lymphoma, retinoblastoma and Wilm's tumor.

        The 174 late side-effects included endocrine disorders (19%), cognitive impairment (14%), orthopedic dysfunction (12%), alopecia (12%), dental damage (11%), psychological (8%) and neurological (8%) disturbances, and azoospermia or amenorrhea (5%). There was no cardiac or renal damage and no second malignancy.

        29% of side-effects were severe. There was significant reduction in quality of life in 54 (49%), in 27 of whom it was severe enough to require psychological intervention. Treatment of brain tumor caused 98 late side-effects in 28 patients (sequelae-to-patient ratio [SPR] 3.3). Most cognitive, endocrine and neurological disorders, and most cases of alopecia, dental and psychological difficulties were in these patients. There were frequent late complications in those treated for retinoblastoma (SPR 1.8), and bone or soft tissue sarcomas (SPR 0.8). Those treated for Wilm's tumor had few side-effects (SPR 0.4).

        Late side effects were most frequent after radiation, reaching as high as SPR 2.4. It averaged only 0.5 in those treated with chemotherapy alone or in combination with surgery.

        Reduction of late side-effects in these patients requires using less toxic modalities, as long as cure rate is not compromised. When considering secondary strategies, screening for early detection of late complications would enable immediate solutions, such as hormonal replacement or providing compensating skills for post-treatment disability.

        The Gift of Breastfeeding

        Breast milk is the optimal food for infant growth and development, the prevention of infectious diseases and mother- child bonding. From the economic perspective, breastfeeding is cost-effective both for the family and society as a whole. The Israeli Ministry of Health encourages breastfeeding as the exclusive source of nutrition for infants in the first 4-6 months of life, with gradual addition of complementary foods thereafter. The promotion of breastfeeding in Israel requires comprehensive national activity with involvement of all the stakeholders. This includes implementation of the International Code of Marketing of Breast Milk Substitutes and joining the international "Baby Friendly Hospitals" project. Knowledge of breastfeeding should be spread, health professionals should be encouraged to become agents of change, support by breast counselors should be encouraged, post-delivery vacations from work should be prolonged and empowerment of women implemented.

        Introduction of Routine Hepatitis Immunization in Israel

        Notification of hepatitis A, which is endemic in Israel, has been compulsory since the establishment of the State. From 1992-98 an average of 2,600 cases were reported annually. Many infections are asymptomatic and mild, especially in children. In general, severity increases with age; in Western countries the case fatality rate is 1.5/1,000 among children less than 5 years old and 27/1,000 among those over 50. Until 1987 incidence in Israel was higher in Jews than in non-Jews, but since 1988 incidence has been about 50% higher in non-Jews.

        Among Jews highest age specific rates shifted from children 1-4 years old to children 5-9 years old in 1970, and in non-Jews in 1989. Improved sanitary conditions and personal hygiene have reduced very early childhood exposure and hence increased the proportion of susceptible older children and adults, in whom symptomatic disease is more prevalent.

        Israel is the first country in the world to include hepatitis A vaccine in its routine immunization schedule. The vaccine is given in 2 doses: at 18 months and 24-30 months of age, and there will be epidemiologic and serologic follow-up. A significant decrease in hepatitis A morbidity is expected in small children within 5 years. The percentage of reported cases in older children and in adults is expected to increase, although the absolute incidence among these groups will decrease.

        Nutrition for Women's Health

        Biological, environmental and social factors predispose women to cardiovascular diseases, malignancy, osteoporosis, diabetes, obesity and eating disorders. Their prevention requires that health services recognize women as a risk group and provide appropriate financial and professional resources. To develop and apply intervention programs for women, funding must be allocated for data collection, development and assessment of intervention programs and involving women in decision processes. We address the diseases and conditions in which nutrition may contribute to primary or secondary prevention of the specified diseases.

        Screening Tests for Detecting Breast Cancer: Performance Rates and Characteristics of Patients

        Screening tests including mammograms for early detection of breast cancer are in the basket of services provided under the National Health Insurance Law. The law also defines the population groups entitled to mammograms, and sets the frequency of the test as once every 2 years. The aim of this study was to examine the screening rates for early detection of breast cancer at the inception of the law, in order to monitor future changes in performance rates, and identify the characteristics of those unlikely to undergo these tests, in order to promote the use of screening tests by them. This study is part of a comprehensive survey examining the effect of the National Health Insurance Law on the Israeli population.

         

        The survey, carried out in the summer of 1995, included a sample of 1,400 Israeli residents aged 22 and over. To determine performance rates for clinical breast examinations a subgroup of 515 women aged 30 and over was sampled. To determine performance rates for mammograms, 173 women were aged 50-74 (the entitlement ages under the law), were sampled. 54% of women aged 30+ reported having had at least 1 clinical breast examination during their lifetime, and 56% of women aged 50-74 reported having had a mammogram during the past 2 years. Variables with an independent effect on the likelihood of having had a clinical breast examination by a physician during the past 2 years were: being over the age of 50, having at least 9 years of education, being a veteran resident of Israel (as opposed to a new immigrant), and being Jewish. Variables with an independent effect on the likelihood of having a mammogram during the past 2 years were: being a veteran resident of Israel, and having a high income צ which was found to have a borderline effect. The findings of the survey emphasize the importance of changing the behavior patterns of both women and physicians, informing women about their entitlement to the tests, and raising consciousness of the importance of having repeated clinical examinations by a physician, as well as mammograms.

        Prevalence Rate, Place of Hospitalization and Source of Referral of Complex Nursing Care Patients in Geriatric Hospitalization

        For the past several years, the hospitalization and care of complex nursing care patients (CNCP) has caused concern among organizers, funders and care-givers of the geriatric hospitalization system (GHS). To assist in improving efficiency of the GHS and to address the lack of comprehensive, empirical and up-to-date information on these patients, we conducted a survey to characterize CNCP, to assess their medical problems and to determine their prevalence among the patients in GHS. The survey was conducted in 1994 using a day census. Data were collected on the 2,319 patients in geriatric beds in all the general hospitals and geriatric hospitals in Israel on the day of the survey.

        28% of the patients in the GHS were CNCP and a quarter of them (7% of all patients) suffered from more than 1 medical condition. Tube feeding was the most prevalent condition (13% of all patients), followed by terminal illness (9%), deep pressure sores (7%) and intravenous transfusion for more than 3 days (6%). The 80% of the CNCP were hospitalized in geriatric hospitals, primarily in geriatric rehabilitation wards. Their average length of stay was over a year. Internal medicine wards of general hospitals were the most frequent source of patient referral. The survey's findings raise issues related to the organization of care of CNCP. They may serve as a basis for the reorganization of the geriatric hospitalization system in order to improve efficiency and quality of care for the benefit of patients, their families and services providers.

        Hyperbaric Oxygen for Carbon Monoxide Poisoning

        Severe cases of carbon monoxide (CO) poisoning from all over Israel are treated at the Israel Naval Medical Institute with hyperbaric oxygen (HBO). Between 1.11.94 and 15.2.95, 24 cases of CO poisoning were treated. Poisoning was usually due to domestic gas-fired heating systems, CO being the only toxin involved. Since delay between termination of CO exposure and arrival at the emergency department averaged 55 minutes, the level of carboxyhemoglobin measured on presentation did not always reflect the true severity of the poisoning. Poisoning was defined as severe and requiring HBO treatment when 1 or more of the following indications was present: evidence of neurological involvement, cardiographic signs of acute ischemic injury, metabolic acidosis, carboxyhemoglobin level greater than 25%, and pregnancy. 20 (84%) recovered consciousness during the course of 1 session (90 min.) of HBO treatment (p2 2.8 ATA) or immediately thereafter, with resolution of other signs of CO poisoning. 3 required a second treatment session before their symptoms resolved. A patient who arrived in deep coma with severe cerebral edema died. HBO is an important element in the combined treatment of severe CO poisoning. There should be greater awareness of the danger of CO poisoning and the means of preventing it, both among medical staff and the population as a whole, mainly in areas in which cold weather requires use of heating systems, which may be gas-fired.
