        שבר הזיז העל-קונדילרי (supracondylar) של עצם הזרוע הוא נדיר ביותר: דווח על חולים ספורים בלבד בספרות הרפואית.

        הזיז העל-קונדילרי (זע"ק) (1) (supracondylar process) הוא בליטה גרמית בהיבט קדמי-פנימי בשליש הרחיקני של עצם הזרוע, המופיע ב-1% אחד בלבד מהאוכלוסייה.

        במאמר זה מובאת פרשת חולה, שלקה בשבר זיז זה הנלווה לשבר באמה (forearm). לנוכח חשיבות המיבנים העוברים בסמוך לזיז ופוטנציאל הפגיעה הנלווה לשבר, מובאת פרשת חולה זה, וכן סקירת הספרות הרפואית בנושא והנחיות לטיפול.

        גאמא הידרוקסיבוטיראט (גה"ב) הוא סם לא מוכר לרופאים רבים. לאחרונה עולה שיעור הצורכים סם זה עקב השפעותיו המרגיעות והמעוררות מינית. אולם השפעותיו של סם זה עלולות להוביל לאי-ספיקה נשימתית ואף למוות. במאמר זה מדווח על הרעלת גה"ב עם סקירת הספרות בנושא, תוך שימת דגש על הפארמאקודינמיקה, ההשפעות קליניות וההלצות לטיפול בלוקים בהרעלת גה"ב.

        Marked Creatine-Phosphokinase Elevation in Myopathy after Treatment with Bezafibrate

        Bezafibrate is a fibric acid derivative which has been widely used in the past 15 years. Recent studies have elucidated much of its mechanism of action, which mainly results in reduction of VLDL and triglyceride levels and in elevation of HDL. The drug is relatively safe and its side-effects well known, mild, and reversible.

        The most severe side-effect is myositis, varying from mild flu-like symptoms to rhabdomyolysis, which is extremely rare. The underlying situations most frequently associated with bezafibrate-induced myositis are renal insufficiency and concomitant treatment with certain other drugs.

        We describe 2 women who developed severe myositis with bezafibrate treatment. 1, aged 43, who had moderate diabetes but no renal insufficiency, was treated with metformin and warfarin, which can interact with bezafibrate and affect its metabolism. The other, aged 54, had renal insufficiency and was on home peritoneal dialysis. Her bezafibrate dose had been increased because of very high triglyceride levels.

        The aim of the study is to call attention to this significant side-effect of benzafibrate and to ways of preventing it.

        Dose-Intensive Chemotherapy with Continuous Infusion 5-Fluorouracil

        54 patients with advanced malignancy refractory to chemotherapy were studied to evaluate efficacy and toxicity of continuous infusion of 5-fluorouracil (5FU) given for 3 weeks. We report results of the first 156 courses given in combination with other drugs.

         

        19 (37%) of the 54 responded, including 3 (6%) with complete response. Toxicity was acceptable, with mucositis in 13 (26%) and 3 (6%) with grade II-III toxicity. Results and toxicity profile were compatible with further disease-oriented studies using this dose-intensive program.

        Vacuum Phenomenon in the Hip Joint: Diagnostic Value

        Vacuum phenomenon is well known in degenerative spinal disease in the elderly, but is seldom seen in other joints, especially in children. The phenomenon does not represent a pathological finding, and can be used for imaging of the articular facets, mainly in the hip and knee joints. We report a patient with this phenomenon in the hip joint.

        Necrotizing Soft Tissue Infection

        The dramatic course of necrotizing soft tissue infection represents a medical emergency, since it is limb - and life-threatening. Most necrotizing soft tissue infections are caused by mixed aerobic and anaerobic Gram-negative and Gram-positive organisms. Most case have been reported in immunocompromised hosts after penetrating trauma or surgery. We describe a unique series of cases of necrotizing soft tissue infection. The mainstay of treatment is early and daily debridement of devitalized tissue and broad-spectrum antibiotics. Hyperbaric oxygen therapy should be considered.

        Topical Photodynamic Therapy in Basal and Squamous Cell Carcinoma and Penile Bowen's Disease

        Photodynamic therapy (PDT) is a noninvasive selective therapy for a specific group of skin tumors. 5-aminolevulinic acid 20% in a water-in-oil cream base was applied to the tumors as a photosensitizer and was followed after 12 hours by exposure to a high output light source emitting red (585-720 nm; 150 mW/cm²) and near infrared irradiation (1.25-1.6 mm; 50 mW/cm²) for 10-15 minutes (VersaLight, incoherent filtered light source). Complete responses were achieved after 1-3 treatments in 26/31 lesions of superficial or small nodular basal cell carcinoma (BCC) and 4/5 in superficial, squamous cell carcinoma (SCC) lesions. There was a patient with Bowen's disease of the penis. Follow-up was for 12-24 months. This noninvasive, nearly painless treatment gives excellent therapeutic and cosmetic results. Our data show its efficacy for certain subtypes of BCC, SCC and Bowen's disease. Further studies will determine the exact cure and recurrence rates with this modality and compare it to other modes of skin cancer therapy.

        Pelvic Lytic Lesion and Osteoporosis-Related Fractures

        Insufficiency fractures of the pelvis may be overlooked as a cause of hip or groin pain. These fractures occur in the elderly, usually those with pronounced osteopenia of the pelvis. Predisposing factors include corticosteroids, local irradiation and postmenopausal osteoporosis. These fractures are difficult to detect clinically and plain radiographs and other studies may be misleading, delaying diagnosis and treatment. A 65-year-old woman had left groin and hip pain for 2 months with no history of trauma. Plain radiographs showed lytic lesions in the left pubic rami. Bone scan revealed increased uptake in that region, suggesting metastatic bone disease. Computed tomography and magnetic resonance imaging demonstrated fractures in the left superior and inferior pubic rami, with callus formation with no involvement of soft tissues. Quantitative computed tomography indicated low calcium concentration, below fracture threshold. The diagnosis of insufficiency fractures of the pelvis was confirmed by the favorable clinical and radiographic outcome. It is therefore important to be familiar with the appearance and location of these fractures.

        Combined Treatment for Adhesive Capsulitis of the Shoulder

        Adhesive capsulitis is a problem for the orthopedic surgeon due to the difficulty of treatment. Although it is self-limited, few patients will wait for spontaneous resolutions while suffering pain and progressive loss of motion. Our aim was to modify the course of the disease and to shorten recovery time by combining intensive physiotherapy with intra-articular infiltration and gentle manipulation. 49 patients with 50 frozen shoulders were enrolled in the study. All patients were treated initially with physiotherapy for 4-8 weeks. If no improvement was noted the affected shoulder was infiltrated and gently manipulated. 27 of 49 patients (55%) improved dramatically with the initial physiotherapy regimen. They achieved full or nearly full range of motion, with significant relief of pain. 22 patients were infiltrated and manipulated. Elevation improved significantly from an average of 110.95 to 165.71 degrees (p<0.001), external rotation from an average of 9.52 to 43.57 degrees (p<0.001) and internal rotation also improved significantly (p<0.001). Self assisted physiotherapy is the corner stone of treatment in adhesive capsulitis. When pain and limitaof passive range of motion persist, infiltration and gentle manipulation dramatically shortens the debilitating process.
