        הוואגיניסמוס הוא הפרעה המשויכת לגינקולוגיה ולפסיכיאטריה. רק חלק מהלוקות בוואגיניסמוס מגיעות לטיפול פסיכיאטרי. ייתכן שמקורה הנפשי של ההפרעה שכיח יותר מהאבחנות בפועל. במאמר זה מובאים הנסיבות והגורמים הפסיכולוגיים שבבסיס תופעה זו, בחמש פרשות חולות. בשלוש פרשות החולות הראשונות – נשים צעירות מרקע תרבותי שמרני או דתי – אובחן ואגיניסמוס ראשוני שגרם לנישואים בלתי-ממומשים. התגלה שלאותן נשים היה "סוד" הקשור לחוויה מינית מוקדמת. פרט זה הוא משמעותי ואף עשוי להוות נקודת-מיפנה בטיפול. שתי פרשות החולות הנותרות הן של נשים מבוגרות יותר שלקו בתופעה של ואגיניסמוס מישני לאחר שנים של חיי-מין ללא ואגיניסמוס. במאמר נדונות דרכי הטיפול, השונות בדרך-כלל, בכל אחד משני סוגי ואגיניסמוס מישני עשויה להתגלות הפרעה משמעותית ברקע האישיותי או הזוגי, שיחייב טיפול נרחב וממושך יותר. נודעת חשיבות רבה לכך שרופאים יתנו את דעתם לאותן נשים הזקוקות לטיפול פסיכיאטרי, ולא גינקולוגי.

        סכיזופרניה היא מחלה הטרוגנית עם מיגוון תסמינים רחב, ללא אטיולוגיה ברורה או אחידה. במהלך השנים, הועלו תיאוריות שונות בניסיון להסביר את ההסתמנות בסכיזופרניה באמצעות דגמים שונים, ומרביתם מבוססים על הפרעה בפעילות מרכזית של נירוטרנסמיטרים שונים, במיוחד דופאמין, סרוטונין וגלוטמאט.

        בתחילה שלטה התיאוריה הדופאמינרגית שהניחה, כי קיימת פעילות-יתר דופאמינרגית במוח בחולים בסיזופרניה, במיוחד גירוי עודף של קולטנים מסוג D2, בעיקר במסלולים מזולימביים ומזוקורטיקליים.

        שיעור האלימות בחברה המערבית בכלל ובישראל בפרט נמצא בעלייה מתמדת, והפך להיות גורם משמעותי לתחלואה ותמותה. האלימות מהווה בעיה חברתית, חוקית וסוגיה בבריאות הציבור. בחודשים האחרונים, מדווח בעיתונות הכתובה והאלקטרונית על מעשי אלימות ורצח של חולי נפש. בחלקם היו החולים בקשר כלשהו עם שירותי בריאות הנפש בעברם. בנוסף, בחלק מהמצבים בהם ננקט פשע אלים, טוען הנאשם לאי-שפיות כהגנה משפטית. בסקירה זו, נדון בשאלה האם חולי נפש בכלל וחולי סכיזופרניה בפרט אלימים יותר משאר האוכלוסייה, והאם הפשע שמבצע חולה הנפש שונה באיפיוניו מפשעים המבוצעים על ידי עבריינים שאינם חולים.

        Prematurity: Interplay between Psychological and Biological Risk Factors Leading to Infant Psychopathology

        Concepts such as risk factor, vulnerability, protective factor and resiliency have become central in the field of developmental psychopathology. The birth of a very-low-birth weight premature baby can be used as a paradigm of the interplay between these factors. Indeed, prematurity implies for both infant and parents, biological as well as psychological risk factors. They may interact in such a way that the child's emotional, cognitive and social development will deviate from normal. Understanding the psychological impact of premature birth includes reference to both the normal psychological processes that characterize pregnancy that are jeopardized by a premature, often traumatic delivery, and to the special significance of being a parent in the Special Care Nursery.

        The contrast between the expected appearance of the baby and that of the sick-looking, tiny premature, together with uncertainty about its medical status, often affect the parents' bonding process. In addition, must learn quickly to cope with issues such as total dependence on a verybusy team, loss of control of the care of their baby, and their unclear roles as parents. Added to these risk factors are the specific neurobehavioral characteristics of premature babies, which often make it hard for parents to read their cues and respond to them properly.

        A clinical vignette illustrates the chain of psychological and biological events that lead to severe disturbance of the early parent-child relationship. It also brings up the question of psychosocial intervention in the Special Care Nursery, both in terms of early detection of families at risk and the types of intervention.

        Psychological Treatment for Paruresis (Bashful Bladder)

        Paruresis, or bashful bladder, is a functional disorder that involves inability to urinate in the presence of others. The symptom meets the DSM-IV diagnostic criteria for simple phobia or social phobia. Although the prevalence of paruresis is relatively high (7% in 1 study and 32% in another), there are few reports of its treatment. The treatment of choice presented in the literature is cognitive-behavioral.

        We describe a multidimensional model used in treating 3 men who suffered from paruresis. Promotion of relaxation, mental imagery, paradox, gestalt, metaphor, cognitive-behavioral and psychodynamic techniques were used. After 5 treatment meetings, 2 younger men, 18- and 24-years old, respectively, were able to overcome the condition, while a 50-year-old got only partial relief.

        Presurgical Neuropsychological assessment in Epilepsy: The Wada Test

        37 candidates for epilepsy surgery underwent the intra-carotid amytal procedure (IAP; also known as the Wada test) to determine hemispheric speech dominance and memory capacity. 31 demonstrated left hemisphere speech dominance, 2 showed evidence for bilateral language and 4 demonstrated right hemispheric language dominance.

        Our study supports a correlational relationship between handedness, lesion laterality and age of onset of seizures, as reported in earlier studies. Left-handed patients with a left hemisphere lesion whose seizures began to an early age had a strong tendency for reversed language dominance.

        Asymmetry of at least 20% in performing the memory test was taken as the cutoff score for demonstrating laterality of lesions. The asymmetry score correctly predicted laterality of lesions in all 28 patients; 6 did not have asymmetry scores and 3 were examined for language only. None of the patients who successfully passed the Wada memory test had any significant postsurgical memory deficits; 1 had transient reduction in verbal memory and 4 who did not pass the test were not operated on for this reason.

        Our results demonstrate the importance of the Wada test in determining cerebral speech dominance, in predicting post-surgical amnesia, and support its usefulness in predicting laterality of seizure focus in candidates for temporal lobectomy.

        Quality Assurance in the Mental Health Department of the Israel Defense Forces

        A review of quality assurance in the mental health department of the Israel Defence Forces allowed the examination of certain unique elements of quality control which pertain to the military. These include the psychiatric medical board, the computerized documentation of appointments and sessions with soldiers, the psychiatric hospitalization database, control systems implemented in the draft boards, peer-review boards and supervision, and a special officer in charge of handling outside consultations and queries. There were other components of quality assurance and control as well. These instruments are vital in a dynamic system constantly striving to improve clinical performance.

        Future plans include the continued use and expansion of quality control boards, the inclusion of quality assurance in the curriculum of mental health officers, and the use of clinical guidelines in working with soldiers. All of this is in keeping with the principle of continuous quality improvement, with the aim of viewing the soldier in need of help as a client.

        Feeding Disorders in Infancy: Feeding Interaction Concept in Diagnosis and Treatment

        In infancy clinical manifestations of psychological distress are mainly somatic. Feeding disorders are one of the most common and nonspecific manifestations of different kinds of disturbed parent-child relationships. These disturbances may have their origins in the baby's constitution and physical status, in the parent's personality structure, or both, as has been conceptualized in the transactional model of normal and abnormal development. Among the daily interactions a baby has with parents, feeding has special inherent impact on the early parent-child relationship because of its psychological meanings. Therefore, feeding disorders, with or without failure to thrive, often reflect various disorders of infancy, still not well recognized in the medical community, such as regulatory disorders, attachment disorders, depression of infancy, disorders of separation-individuation, and post-traumatic eating disorder. 3 clinical cases are brought to increase awareness of psychological distress in the infant, and of feeding disorders as 1 of its manifestations. Each illustrates a different kind of feeding disorder in terms of etiology and pathogenesis. Through these cases we emphasize the need for a multidisciplinary, integrative approach in diagnosis and treatment. Our conceptual background is based both on the transactional model of development (infant and parental factors impact on each other) in a very dynamic paradigm, and on psychodynamic premises. Intrapsychic conflicts and past representations impact heavily on the parenting characteristics. We emphasize the psychological significance of disturbed feeding interactions, with or without failure to thrive.

        Characteristics of Israeli Primary Health Care Physicians who Treat Depression

        Primary health care physicians have a pivotal role in treating mental health problems. We determined the proportion of primary care physicians in Israel who treat depression and their characteristics. The study was based on a stratified national random sample of primary care physicians (n677, response rate 78%). From these physicians' reports 22% always treat depression, 36.6% usually, 28.6% sometimes, and 12.6% never. Based on a logistic regression model the physicians who always or usually treated depression were distinguished from the other physicians by their treating more medical conditions on their own, seeing themselves as having more first contact for psychosocial problems, having frequent contact with social workers and specializing in family medicine. Primary health care physicians play a major role in treating depression on their own. This raises new questions about how they treat depression themselves, and under what circumstances they treat or refer to a specialist.

        Suicidal Calls to a Telephone Emergency Service

        Suicidal threats and suicidal ideation among callers to the Israeli hot-line (ERAN) were investigated, using 2 methods of analysis. The first, an epidemiological survey based on 8,272 calls received by all 8 ERAN posts during 2 years, reported on their standard form for recording interactions with suicidal callers. The second is content analysis of 42 detailed reports of interactions with such callers. Epidemiological analysis revealed a higher rate of suicidal ideation among callers with a psychiatric history, compared with those without a history. However, the 2 groups did not differ in rate of suicidal threats. In addition, suicidal threats of adolescents were related mainly to problems of identity and self-image, while mental disorders were the main problems among adults. Content analysis identified 2 patterns of terminating the interaction, which represent 2 strategies of intervention to prevent suicide. The first is establishing a contact between the caller and an appropriate individual in the caller's environment who undertakes responsibility to help the caller. The second strategy is reducing the caller's tension, leading to a withdrawal of the suicidal threat. The first pattern is more characteristic of interactions with psychiatric callers, while the second is more frequent among non-psychiatric callers to ERAN. The hot-line makes a unique contribution in helping those threatening suicide, which constitutes a real emergency.

        Characteristics of Adults with Emotional Distress, and Patterns of Mental Health Services Use

        We sought firsthand data on the extent of perceived mental health needs and on patterns of use of mental health services among Israelis aged 22 and over. The data are from a national survey conducted in 1995. A random sample of phone numbers from the telephone company's computerized listings yielded 1,395 completed questionnaires (response rate, 81%).

        At some point in their lives, 27% had experienced emotional distress or mental health problems with which they had difficulty coping alone; 13.4% reported that they had such an experience during 1995. According to multivariate analysis, those more likely to report mental health problems were women, those with a chronic disease, Russian immigrants, divorced or widowed adults, those with a low level of education, and members of the Clalit sick fund. 38% of those who had ever had emotional or mental health problems had asked for help. The proportion of those seeking help was high among respondents aged 35-55, Hebrew speakers (compared to speakers of Russian or Arabic), and city dwellers, and the rate was low among members of the Clalit sick fund. Of those who did seek help, 39% went to a psychologist or a psychiatrist, 25% to their family doctor, 19% to a family member or friend, 7% to a social worker or social service agency, 6% to other medical personnel, and 4% to a psychiatric hospital. 30% turned for assistance to the private sector and 70% to the public sector.

        These findings have special significance in view of the impending reform of the mental health services. As mandated by the new National Health Insurance Law, mental health services are to be included in the basket of health services provided by the sick funds. The data can be of use in the management of sick funds and for physicians working in the community, as they prepare for this change. In addition, the data will be of aid to national policy makers in planning services suited to the needs of different population groups and to allocate resources more rationally.

        The Homeless and the Health System: Profile of the Homeless Patient

        The homeless population is mobile and does not use ambulatory health care services. Thus the major contact between the homeless and the medical establishment occurs primarily when they are treated for acute symptoms in hospital. We describe the clinical and sociodemographic profile of the homeless who require hospital services. The research population included 50 homeless treated in the emergency room and various departments of our medical center between October 1994 and August 1995. Social workers used a questionnaire relating to clinical, sociodemographic and social factors. Most patients were men, 76% under the age of 50. The most common diagnosis was alcoholism; other diagnoses included back, limb and joint injuries, infections, skin diseases, and general exhaustion. There were subgroups with differing needs within this homeless population for which appropriate rehabilitation programs are proposed.
