        לישמניאזיס בעור, המוכר בישראל בשמו העממי "שושנת יריחו", הוא מחלת עור המתבטאת בהופעת פצעים מכוייבים בקוטר של עד מספר ס"מ, המתקיימים חודשים ארוכים למרות טיפולים שונים. המחלה נגרמת מחדירה של טפילים חד-תאיים (protozoal) מסוג לישמניה לעור בעקבות עקיצה של נקבת זבוב החול. לכן הפצעים ממוקמים לרוב על חלקי גוף שבדרך-כלל אינם מכוסים, כמו הפנים והגפיים. המחלה נחשבת אנדמית באזורים מסוימים בישראל, בעיקר ביקעת הירדן ואזור יריחו, ים המלח, אזור כפר אדומים, רמת הנגב (קציעות) והערבה. בישראל מדובר בבעיה בריאותית משמעותית, הפוגעת בעיקר במטיילים באזורים אנדמיים (חלק גדול מהם חיילים). נפגעים נוספים הם תרמילאים, הנדבקים במחלה במהלך טיליהם, בעיקר בדרום-אמריקה. היעילות של הטיפולים הקיימים כיום כנגד מחלה זו מוגבלת ולרוב אינה נוחה או נעימה לחולים. בשנים האחרונות מתרבים הדיווחים על טיפולים חדשים בלישמניאזיס מערכתי, הנושאים עימם תקווה לגילוי טיפול יעיל גם כנגד לישמניאזיס בעור. בסקירה זו אנו דנים במחלה ובאפשרויות הטיפול. בין השאר, מדווח בסקירה זו על יעילותו של טיפול מקומי חדש ללישמניאזיס בעור באמצעות תכשיר מיוחד של אמפטריצין (amphotericin).

        Secondary Bone Grafting in Cleft Lip and Palate

        Results of reconstruction of residual alveolar bone defects in 52 patients operated between 1990-1998 were evaluated clinically and radiographically in a retrospective study.

        Ages ranged between 9-37; 30 were males. The donor site of bone grafts in all was particulate cancellous marrow from the anterior iliac crest. 32 had unilateral clefts and 20 bilateral. Total cleft sites treated was 72.

        Best results were achieved when bone grafting was carried out prior to the eruption of the canine tooth. The cleft space was closed and oro-nasal fistulas were eliminated in 42 (80%). Success rates in unilateral and bilateral cases were significantly different.

        Autologous Chondrocyte Transplantation - from Science Fiction to Routine Clinical Practice

        Adult articular cartilage lacks the capacity for self-repair. The limiting factor appears to be the inability of chondrocytes to proliferate while embedded in the extracellular matrix typical of hyaline cartilage. Cartilage defects larger than 1 cm2 change articular biomechanics and lead to eventual osteoarth-ritis and joint destruction.

        During the past decade, several competing techniques have evolved to stimulate articular cartilage repair. Small lesions can be successfully treated by either micro-fracture or osteochondral cylinder grafting. The latter technique allows immediate weight bearing but leads to damage of previously uninvolved areas of articular cartilage, which limits its application to lesions of less than 2 cm2.

        When the damaged area is more extensive, grafting of autologous chondrocytes should be considered. First a diagnostic arthroscopy is performed to assess the damaged area and a small cartilage biopsy is taken. 6 weeks later, arthrotomy and chondrocyte transplantation are performed. In the interval, the antologous chondrocytes have expanded by 2 to 3 orders of magnitude. Our experience to date includes 10 cases with follow-up of 6 months to 5 years. Preoperative complaints of crepitation and locking disappear. There is functional improvement and pain reduction of approximately 50%. This procedure, currently limited to patients under 55 years of age with limited damage to an articular surface, for the first time allows reconstruction of damaged articular areas without resorting to allografts.

        Sport-Related Maxillofacial Fractures

        The records of 537 patients with 750 maxillofaciafractures were reviewed and analyzed. 55 (10.2%) had sport-related injuries and the rest were due to othercauses. The sport-related group was predominantly male (ratio 9:1) with a mean age of 24.5 years. The mandible was most commonly injured (52.5%), followed by the zygomatic complex (32.8%). The incidence of complicated mid-facial fractures was only 2.8%, and of comminuted fractures 9%. This distribution of injuries is most likely due to the relatively low-energy of trauma associated with many sport activities. The relatively low mean age of the patients, most of whom had full dentition, allowed for conservative treatment of most of the mandibular fractures, using closed reduction.

         

        Injuries were caused by 11 different sports. The highest incidence was soccer (45%), followed by skating (15%), basketball (9%) and horse-riding (9%). Contact sports were the cause of injuries in 72.2%, with the highest incidence of trauma due to impact with another player (60%). Better protection of the oral and maxillofacial region is needed, especially during high-contact sport.

        Clozapine in the Treatment of Schizophrenia

        Schizophrenia is associated with brain abnormalities and is typically evidenced by disorganized speech and behavior, delusions, and hallucinations; it usually requires extended hospitalization. Its incidence in the western world is estimated at 4-7 cases/10,000/year.

        A method of shortening hospitalization and improving level of functioning is the use of unique medication, including clozapine, which has been in use in Israel for the past 6 years.

        We report 327 patients who participated in a community rehabilitation program and were treated with clozapine. They were compared with 417 patients who corresponded to the guidelines of the Director of Mental Health Services for treatment with clozapine, but were not treated with it for reasons not defined in the guidelines.

        The study included those 25-44 and 45-64 years of age and according to the division of the population of patients hospitalized in both government and private hospitals. The project demonstrates the savings from use of clozapine as opposed to the alternative of hospitalizing these patients. It also shows the complexity and difficulty in assimilating new technologies, in relation to the influence of social considerations and supplier/insurer accounting on the patterns of technological assimilation.

        Cyclic Vomiting Syndrome in Children

        Cyclic vomiting syndrome in children is a manifestation of various etiologies, including gastroenterological and renal disorders, central and autonomic nervous system abnormalities, as well as metabolic and endocrine dysfunction. Frequently no organic cause is found. Personality profiles of children with cyclic vomiting reveal perfectionism, competitiveness, and aggressive behavior. Vomiting attacks have been induced by anxiety and excitement in patients with cyclic vomiting. We describe an 8-year-old girl with cyclic vomiting, frequently associated with occipital headaches, photophobia or dizziness. Psychiatric evaluation indicated a generalized anxiety disorder.

        Distraction Osteogenesis for Hypoplastic Facial Bones

        Distraction osteogenesis is a well-known method for bone lengthening which stretches callus to generate new bone in the distracted area. The method was developed by Ilizarov for the lengthening of long enchondral bones. In recent years the method has also been applied to the facial bones and to the jaw.

        On the Value of the Placebo: Reflections on Homeopathy

        A case is presented in which homeopathic "treatment" helped, despite the fact that the patient did not take the medication. The author surveys research which has been conducted on homeopathy and concludes that its benefits derive from non-specific influences of the healer upon the patient, similar to a placebo effect. The importance of this influence is stressed, and its wider application in conventional medicine is urged.
