חקלאי בן 64, שלקה בסרטן עור רב-מוקדי של הפנים, הסתגר בביתו והתנגד במשך כ-12 שנה לפנות למעקב ולקבל טיפול רפואי, השאת גרמה להרס נרחב ובלתי-שגרתי בחומרתו של הפנים. להפרעה קשה בתיפקוד ובסופו של דבר למותו של החולה. ייתכן שהשאת היתה הישנות של סרטן תאי קשקש, אשר נכרתה בניתוח 12 שנה קודם לכן.

אין אמצעים חוקיים לשלול את זכותו של החולה לסרב לטיפול. גם בידיעה שהזנחת הטיפול במשך שנים תגרום לסבל קשה ולמוות. בנוסף לכך, למעבידים ולמודות המבטחים יש יכולת מוגבלת בלבד לאכוף על עובדים החשופים לשמש את ההסתייעות באמצעי הגנה מפני סרטן העור.

Metallic Stents for Acute Colonic Obstruction

The traditional treatment of acute colonic obstruction, usually caused by malignant tumor, is a 2-stage surgical procedure. The first operation is emergent and includes primary tumor resection and end colostomy followed by scheduled colostomy closure. These operations are associated with high mortality and morbidity. Recently, insertion of self-expanding metallic stents for temporary colonic decompression has been was introduced. With this new technique colostomy can be avoided in the acute phase.

In advanced colonic cancer stent-insertion is the only palliative treatment. It is done in the radiology department under fluoroscopic guidance. We present 2 cases of malignant colonic obstruction treated successfully by stent insertion.

Malignant Nodular Hidradenoma (Sweat Gland Tumor)

Malignanhidradenoma is a very rare tumor that originates from sweat glands. We present a 61-year-old man with an ulcerated tumor in his right flank, 4 cm in diameter, that was excised with a wide free margin. Histopathologic study showed an ill-defined, epithelial neoformation, formed by lobules of clear polygonal cells in the deep dermis and subcutaneous tissue, diagnosed as malignant nodular hidradenoma.

1.5 years after excision there was enlargement of the right axillary and inguinal lymph nodes, which showed metastatic, adnexal neoplastic cells. Axillary resection and superficial dissection of the right inguinal nodes were performed. After 3 months the tumor had spread to other lymph nodes and acute obstructive renal failure required insertion of a pig-tail catheter into the right ureter. Radiotherapy was followed by chemo-therapy, but he died from end-stage metastatic disease in multi-organ failure.
