טרטוקארצינוסארקומה ממאירה של חלל האף וגיתות האף היא שאת נדירה מאוד ופולשנית. היא מתאפיינת בעירוב מרכיבים אפיתליים ומזאנכימיים במישלב עם מרכיבים תאיים בעלי מאפיינים עובריים.

במאמר זה מדווח על פרשת החולה הראשון והיחיד בישראל שלקה בשאת מסוג זה  (בספרות הרפואית בעולם דווח עד היום על 40-50 פרשות חולים שלקו במחלה זו).

Verrucous carcinoma היא שאת נדירה מהסוג של שאתות קשקש ממאירות – squamous cell carcinoma. האזור האופייני להתהוות שאתות מסוג זה הוא בחלל הפה והלוע. בדרך-כלל מתהוות השאתות ברירית של החניכיים והלחיים. מובאת בזאת פרשת חולה של גבר בן 70, שהופנה למרפאתנו עקב קושי בנשימה דרך האף. בבדיקה נמצא גוש ממאיר באף, שאובחן כשאת ממאירה מסוג verrucous carcinoma. למיטב ידיעתנו, זהו הדיווח הראשון והיחיד בישראל על שאת מסוג זה באף.

Management of Nasal Meningoencephaloceles and Cerebrospinal Rhinorrhea

CSF rhinorrhea constitutes a diagnostic challenge. If unrecognized or incompletely managed, it can result in devastating complications. The physician must e aware to this entity and it's management. The conventional neurosurgical management of meningoencephaloceles and cerebrospinal rhinorrhea has been by the intracranial approach.

Otolaryngologists have undertaken extracranial approaches for repair of these problems with fair results. In recent years, functional endoscopic sinus surgery has gained polularity and was advocated for the repair of nasal meningoencephaloceles and CSF fistulae. Between 1998 and 1999, five patients were operated by the senior author (M.P) by means of endoscopic sinus surgery.

High success rate and lower morbidity make this approach the treatment of choice. The perioperative use of fluoroscein allows us to locate precisely the defect and to confirm complete sealing of the leak.

We present our experience in managing 5 cases, 3 of which presented with meningoencephaloceles.

Nasopharyngeal Colonization with Streptococcus Pneumoniae in Pediatric Respiratory Infections

During the winter of 1995, nasopharyngeal colonization of Streptococcus pneumoniae was evaluated in 204 children with respiratory infection and 107 normal control children. There was no difference in gender or mean age between the groups, and no difference in carrier rate between sick (24.5%) and normal (22%) children (p=0.6). Carrier rates were 19%, 32%, 31% and 17% at 6, 12, 24 and 48 months, respectively. Penicillin-resistant pneumococci (PRP) were found in 42% of sick and 16.6% of normal children, (p<0.05). Resistance to more than 2 antibiotics was found in 28% of sick and in 12.5% of normal children. PRP were found in 67% and 34% of sick children with and without prior antibiotic treatment (p<0.05). We conclude that there is no difference in the carrier rate of Streptococcus pneumoniae between sick and normal children. However, the high prevalence of PRP in children with respiratory infections is probably due to prior antibiotic treatment.
