        מערך רישום הטראומה של ישראל מרכז נתונים לגבי פצועי תאונות דרכים המתקבלים מ-8 מרכזי טראומה בישראל. המידע ברישום כולל פרטים דמוגרפיים אודות הפצועים, פרטי הפציעה (סוג הפציעה ונסיבותיה), מדדים לחומרת הפציעה ומאפייני הטיפול והאישפוז.

        במאמר זה מובא עיבוד הנתונים מרישום הטראומה שנכלולו בו נתוני 4,328 נפגעי תאונות דרכים בשנת 1999. המימצאים מעידים על סיכון גבוה לפגיעות קשות ואף קטלניות בקרב הולכי-רגל (בעיקר ילדים וקשישים), ילדים רוכבי אופניים ונהגי רכב צעירים. מחצית מהפגיעות היו פגיעות מרובות. עם שיעור גבוה של שברים בגפיים ברוכבי אופנוע ופגיעות ראש בקרב הולכי רגל ורוכבי אופניים. נתוני רישום הטראומה מאפשרים בחינה מעמיקה, יותר מתמיד, של אופי הפגיעות מתאונות דרכים ודרגת חומרתן. המימצאים משמשים בסיס לאיפיון הפגיעות והנפגעים ומהווים יסוד לתוכניות מניעה והתערבות, שתופנינה לקבוצות המצויות בסיכון גבוה, כגון קשישים הולכי רגל, ילדים רוכבי אופניים ונהגי רכב צעירים.

        חולים הלוקים בסוכרת מסוג 2 מצויים בסיכון של פי 2-4 ללקות בטרשת-עורקים ובאוטם שריר-הלב ובשכיחות-יתר לתמותה בעת אירוע של אוטם שריר-הלב ואחריו. שיעור התמותה ממחלת לב בחולי סוכרת דומה לזו של אוכלוסייה שאינה לוקה בסוכרת, ושחוותה אוטם בעבר. ב- 80%-75% מהלוקים בסוכרת מתים מטרשת-עורקים וסיבוכיה. סוכרת מהווה גורם סיכון עיקרי למחלת לב ולטרשת-עורקים, ואחד הגורמים העיקריים התורמים להיותה כזו הן ההפרעות במשק השומנים, ההמלצות המובאות בזאת מהוות השלמה להמלצות למניעת מחלות לב, שפורסמו מעל דפי "הרפואה" בגיליון הראשון של המילניום.

        The Lack of Awareness of the Israeli Population Regarding Gastrointestinal Complications from Non-Steroidal Anti-Inflammatory Drugs

        Non-steroidal anti-inflammatory drugs (NSAID) are potent analgesic agents. They are indicated for rheumatoid arthritis, osteoarthritis, severe acute and prolonged chronic pain. A wide range of analgesic agents are available. The objective of this study was to assess the patient's knowledge of his illness, his knowledge familiarity with NSAID medications and their side effects and thereupon, to evaluate the awareness, utilization, satisfaction and expectations associated with current available treatment.

        During the year 2000, a randomized telephone screening questionnaire, was completed over a 2 month period for a sample group of the general Israeli population over 50 years of age.

        Of the 3179 persons contacted, 2028 responded and 292 persons were treated with NSAIDs and further investigated. Fifty percent suffered from chronic musculoskeletal pain, 22% had osteoarthritis, 10% had rheumatoid arthritis, and 18% had undefined chronic pain. The average age of the respondent was 65 years old. The average duration of treatment was 11 years. The most common drugs used by the 292 respondents that were taking NSAIDs were diclofenac (Voltaren) - 51% and naproxen (Naxyn) - 21%. Only 4% were treated with COX-2 specific inhibitors. Twenty percent of the respondents utilized more than one NSAID simultaneously. Forty seven percent of the patients had peptic ulcer disease but only 39% of them were simultaneously treated with an NSAID and a gastroprotective agent. Sixty eight percent of patients expected amelioration of symptoms while taking NSAIDs. Seventy seven percent of respondents were dissatisfied with treatment. The patients expressed that they did not have a reliable easily accessible source of information on their disorder or on available treatment options.

        Many respondents were not aware of their medical diagnosis. In general, patients were not aware of all the available NSAIDs or the side effects of gastrointestinal complications. Enhancement of information available to the patient is warranted to improve the provision of efficient therapy, reduce side effects and increase patient satisfaction from therapy.

        פרס נובל בפיסיולוגיה או ברפואה לשנת 2000 הוענק במשותף לשלושה חוקרים: ארביד קרלסון, פול גרינגארד, ואריק קנדל, על תגליותיהם בשטח "העברת אותו במערכת העצבים".

        במוח האדם יש יותר ממאה מיליארד תאי עצב, המחוברים ביניהם ברשת ענפה ואינסופית של חיבורים. העברת אותות בין התאים נעשית בתיווך של חומרים כימיים, המגשרים על הסינפסות המפרידות בין התאים. לכל תא יכולות להיות אלפי נקודות מגע כאלה עם תאי עצב אחרים. שלושת חתני הפרס גילו תגליות חשובות הקשורות בהעברת האותות בין התאים בתהליך שנקרא העברת אותו איטית. תגליות אלה היו חיוניות להבנת התיפקוד הטיבעי של המוח, וכיצד הפרעות בהעברת אותו אלו עלולות לגרום למחלות במערכת העצבים ולמחלות נפש. בצד מימצאים אלה, פותחו תרופות לטיפול בהפרעות כנ"ל.

        Retinal Injury Induced by Laser Pointers

        Laser pointers originally designed for use during presentations are ubiquitous and are even sold as toys (such as pens or on key chains) in drug stores. Though reported as safe, the laser pointers still carry the risk of potential damage to the eye. We report a 16-year-old boy with bilateral retinal injury caused by 20-30 seconds of exposure to a laser pointing-device. Immediately thereafter, vision was blurred bilaterally and he noted a central red scotoma in each eye. Symptoms resolved spontaneously within 2 days but the retinal scars remained all during the 10 months of follow-up.

        It is clear from our report and 3 other publications that retinal damage can develop from misusing laser pointers. Laser hazards and safety should be stressed for the general public. We recommend that laser-pointers should not be available as toys to children and teenagers.

        Folic Acid for Preventing Neural Tube Defects

        Spina bifida and anencephaly are the most common, serious malformations in neural tube defects (NTD). Randomized trials in the last 2 decades have demonstrated that folic acid, 0.4 mg/d, reduces the incidence of NTD by more than 50%. We investigated the use of folic acid and multivitamins containing folic acid in childbearing women.

        Of 221 women interviewed, 67 (30%) regularly took pills containing 0.4 mg folic acid. Women with higher educational levels were more likely to take multivitamins with folic acid than were the less educated (p=0.05). Of the women who took folic acid, only 5 (7.5%) used separate folic acid tablets, before and during their pregnancy. The rest used multivitamins containing folic acid. The 5 women who took folic acid separately were college-educated and nonreligious, and they took multivitamins in addition (p>0.05).

        Of the women interviewed, 58 (26.2%) were Bedouin of the Negev. 24 (41.4%) of them took pills containing folic acid on a regular basis. This percentage is higher than that in the Jewish women in the study who took folic acid for prevention of NTD (17%; p=0.038).

        Most of the women took folic acid after the first trimester. Only a minority took daily periconceptional folic acid. Multivitamins containing 0.4 mg of folic acid were more popular than folic acid tablets alone. This study emphasizes the need for continuing efforts to increase consumption of folic acid and awareness of its benefits among women of childbearing age.

        Cilazapril for Essential Hypertension Treated in the Community

        In a multicenter study in community clinics, 413 patients with mild to moderate essential hypertension were treated with cilazapril (Vasocase), 2.5 mg daily. Patients had either been untreated or had developed side-effects from previous antihypertensive treatment. When response was inadequate the dose was either increased to 5 mg or another antihypertensive medication was added, or both.

        Treatment significantly reduced systolic and diastolic blood pressures. Pulse rate decreased significantly from the second month of treatment onwards. At the end of the 3rd month of treatment blood pressure was normalized or had decreased by more than 10 mmHg in 91.9% of patients. Physicians' evaluations revealed improvement in 62%; patients' self-evaluations suggested improvement in 61%. Efficacy was equal in all age groups and in both obese and nonobese patients. Antihypertensive response was superior in those with normal renal function. Side-effects were rare and similar to those reported in the literature.

        Incidence of Insulin Dependent Diabetes in Youth in Israel

        Recent reports from different countries have shown an increasing incidence of insulin-dependent diabetes mellitus (IDDM, type I diabetes). In Israel, several surveys of the incidence of IDDM have been conducted. During 1996, a national juvenile diabetes register was founded by specialists in endocrinology, and the Israel Center for Disease Control (ICDC).

        This is the first report of the national incidence of IDDM in the 0-17 year age group in Israel. New cases of juvenile diabetes were reported using an anonymous form. The ICDC was responsible for data collection, control and statistical analyses.

        During 1997, 162 new cases of diabetes mellitus were reported in the age group 0-17, 154 of them diagnosed as IDDM. The annual incidence in the total population of Israel in 1997 was 7.7/100,000. Age-specific incidence was 7.3/100,000 in boys and 8.1 in girls. A family history of IDDM was found in 14.3% of the cases. Incidence was higher for Jews (9.2/100,000) than Arabs (3.6/100,000). Among Jews, children whose fathers were born in Yemen had the highest incidence and those born to fathers born in Israel the lowest incidence.

        IDDM incidence rates for 1997 are higher than reported in previous surveys. It will be possible to draw conclusions regarding possible trends in incidence from data to be gathered in the next few years.

        Treatment of Hypertension with Losartan

        The efficacy, safety and side-effects of treatment with losartan (Ocsaar) was studied for the first time in Israel in a large group of patients with mild to moderate hypertension in several community clinics. The 421 patients (51% men) aged 30-86 years (mean 58.6) received 50 mg of losartan daily, increased when necessary to 100 mg, and/or a second antihypertensive drug was given. After 4 weeks blood pressure was normalized in 344 and after 12 weeks in 363. Side-effects were minimal and treatment was effective in all age groups.

        Acute Malaria in an Israeli Tourist to Kenya

        Malaria is 1 of the main causes of death in third world countries. It has become extinct in Israel and imported cases are rare, since most visitors to endemic countries take anti-malarial prophylaxis. We report an Israeli tourist to Kenya infected with falciparum malaria complicated by severe metabolic acidosis, renal failure and adult respiratory distress syndrome. After intensive care treatment this preventable condition improved.

        Characteristics, Management and Prognosis of Acute Myocardial Infarction (Israel 1990-1996)

        From 1990 to 1996 we conducted consecutive 2-month surveys in all CCUs (n=26) in Israel. The aim was to compare the characteristics, management, and prognosis of patients with acute myocardial infarction (AMI) between the 4 surveys.

         

        Patient characteristics were similar in all surveys. About 75% of patients were males and had a first MI. Mechanical and arrhythmic complications decreased slightly between 1990 and 1996. In contrast, frequency of treatment with thrombolysis, aspirin, beta-blockers and ACE-I, as well as coronary interventional procedures, increased tremendously.

        In parallel, 30-day and 1-year mortality decreased significantly, from 16.5% and 22.5%, respectively, to 9.0% and 13.9%. After multiple adjustment for factors associated with mortality, the 30-day relative risk of death for patients hospitalized in 1992, 1994 and 1996 was 0.79 (95% CI 0.59-1.08), 0.75 (95% CI 0.56-1.01) and 0.54 (95% CI 0.39-9.74), respectively, as compared with 1990.

         

        Although there is no direct proof that changes in management of AMI are related to the reduction in mortality seen during the course of the surveys, the association seems likely.

        The Homeless and the Health System: Profile of the Homeless Patient

        The homeless population is mobile and does not use ambulatory health care services. Thus the major contact between the homeless and the medical establishment occurs primarily when they are treated for acute symptoms in hospital. We describe the clinical and sociodemographic profile of the homeless who require hospital services. The research population included 50 homeless treated in the emergency room and various departments of our medical center between October 1994 and August 1995. Social workers used a questionnaire relating to clinical, sociodemographic and social factors. Most patients were men, 76% under the age of 50. The most common diagnosis was alcoholism; other diagnoses included back, limb and joint injuries, infections, skin diseases, and general exhaustion. There were subgroups with differing needs within this homeless population for which appropriate rehabilitation programs are proposed.

        Cellular Phone Interference with Medical Instruments

        Cellular telephones and other telecommunication equipment occasionally cause malfunctioning of medical equipment, including life-support equipment. We review such malfunctioning and relate it to Israeli and worldwide standards, analyzing the characteristics of the interference in terms of amplitude and frequency. The results of a controlled study of interference by cellular telephones and portable 2-way radios with medical devices in our clinical departments are also presented. The levels of background environmental electromagnetic noise at several sites in both Hadassah hospitals (Ein Karem and Mount Scopus) were measured, as well as signal levels of cellular telephones and other communication equipment at various distances and in various areas. We recommend 2 different levels of restrictions on the use of this equipment within the hospital.

         

        These include prohibition of the use of wireless telecommunication equipment in intensive care areas and operating theaters. In all other areas it is recommended to turn off the cellular telephone within 1 meter of medical devices and not to transmit (but only to receive) calls with a portable 2-way radio within a 5 meter distance of medical devices.
