Twenty-Five Hours of Fasting with Automated Insulin Delivery in Youth with Type 1 Diabetes

This real-world observational study evaluated adolescents and young adults with type 1 diabetes fasting for 25 hours (Yom Kippur) while using automated insulin delivery (AID) systems. Participants successfully completed the fast with improved time-in-range and reduced hypoglycemia compared to usual days, and no severe hypoglycemia or serious adverse events occurred. Insulin doses were often lower than usual, and modest adjustments to AID settings were typically sufficient. The findings suggest that AID systems can help maintain safe glucose control during prolonged fasting.
