تحديث البيانات الشخصية

اذا طرأ أي تغيير على تفاصيلكم الشخصية، يمكنكم تحديثها بنفسكم عبر موقع النقابة أو التطبيق.

في الموقع الالكتروني: المساحة الشخصية->الملف الشخصي ->تحديث التفاصيل الشخصية.

عبر التطبيق: تطبيق النقابة الطبية->أدوات لخدمتك ->المساحة الشخصية.

*أوقات تحديث التفاصيل الشخصية مختلفة.  رقم الرخصة، موضوع التخصص، اللقب الأكاديمي، تغيير الإسم ووفاة الطبيب تحتاج إلى التأكيد،  لذلك،  سيتم تحديثها في أقرب وقت ممكن.  أما التفاصيل الشخصية، رقم الهاتف، العنوان ومكان العمل يتم تحديثها في اليوم التالي.

الأطباء الذين طرأ لديهم تغيير في أحد التفاصيل التالية، عليهم الإبلاغ للنقابة الطبية عن ذلك، التفاصيل هي لاستخدام النقابة الطبية فقط.
 

  1. إبلاغ عن الخروج في عطلة: يجب إبلاغ مركز الخدمات التابع للنقابة الطبية على الرقم 03-6100471  عن الخروج في عطلات لفترات طويلة ليكون بالإمكان الإستمرار بدفعات رسوم العضوية على التوالي والحفاظ على حقوق التأمين المعطاة في إطار العضوية.  أمثله على العُطل الطويلة: اجازة الولادة، إجازة بسبب المرض المستمر والإجازات غير المدفوعة.
  2. الإبلاغ عن السفر للخارج: الأطباء المسافرين لفترة متواصلة لأكثر من 3 اشهر عليهم الابلاغ عن ذلك خطياً على العنوان:  النقابة الطبية،  قسم الخدمات،  بناية التوأم 2، شارع زبوطنسكي 35، ص.ب 3566، رمات چان 5213604.  نرجو منك ذكر موعد الخروج للعطلة وموعد العودة المتوقع وذلك للمحافظة على الحقوق في نقابة الأطباء، الأطباء المتواجدين خارج البلاد ملزمين بدفع رسوم الاشتراك كما هو محدد من وقت لآخر.
  3. تحديث لقب الطبيب من دكتور إلى بروفسور: الطبيب الحاصل على لقب البروفيسور من إحدى الجامعات في إسرائيل ومعني في تحديث هذا اللقب في النقابة الطبية،  عليه إرسال نسخة من موافقة الجامعة عن تلقي هذا اللقب إلى  البريد الالكتروني  التابع لقسم الخدمات في النقابة الطبية.
