דרמטיטיס אטופית היא אחת המחלות הדלקתיות השכיחות ביותר במדינות המערב. גורמים רבים מעורבים בפתוגנזה של דרמטיטיס אטופית, אם כי במהלך השנים האחרונות ניכר התפקיד הראשוני של הפרעה במחסום האפידרמי בפתוגנזה של דרמטיטיס אטופית בפרט והתהלוכה האטופית בכלל, דרך עלייה בחדירות אלרגנים לעור עם עירור תגובה דלקתית שניונית. האפידרמיס של חולים עם דרמטיטיס אטופית מדגים הפרעה בכל אחד ממרכיבי המחסום האפידרמי, לרבות חלבונים תוך תאיים (כגון פילגרין, לוריקרין) ושומנים בין תאיים (כגון צרמידים וחומצות שומן חופשיות) והמטבוליזם שלהם, קשרים בין תאיים (כדוגמת tight junctions), ותהליך נשירת תאים בשכבה החיצונית (desquamation). חקר הפגמים בתקינות המחסום האפידרמי והבנת התהליכים הדרושים לשימורו, חיוניים לשיפור הטיפול בחולים עם דרמטיטיס אטופית ולמניעת הופעת הצעדה האטופית בהמשך. במאמר זה נעמוד על החידושים בהבנת המכניזם הפתולוגי של הפרעות במרכיבי המחסום האפידרמי בדרמטיטיס אטופית, על כל ההשלכות הטיפוליות הנובעות מכך.

Dermatitis from Contact with Agave Americana

Various plants induce dermatitis in man. There have been only a few published cases of contact dermatitis caused by Agave americana (AA).

We report intentional exposure to AA in a soldier seeking sick leave, and review our previously reported cases. Treatment with oral antihistamines and topical saline compresses resulted in subsidence of the systemic symptoms within 24h and regression of cutaneous manifestations in 7-10 days.

Physicians should be alert to the possibility of self-inflicted contact dermatitis induced by exposure to plants, especially to A. americana. Systemic signs may accompany the cutaneous lesions.

For several years health funds in Israel have allowed patients to see dermatologists, gynecologists, orthopedic surgeons and ENT specialists without being referred by their primary care physician. There is heated debate about whether this practice is justified and if it is cost effective. Is it better medicine for the patient? What are the roles of the primary care physician and of the primary dermatologist in the care of skin diseases? What do patients want? We examined the practices of 4 dermatology clinics and discuss the issues involved.
