        רקע: בשנים האחרונות גדל והולך בבתי-החולים בישראל השימוש בכפפות לטקס, בעקבות עלייה ניכרת בשיעור המחלות הזיהומיות המועברות דרך דם והפרשות, כגון דלקת כבד מסוג B ו-C ומחלת האיידס. אולם השימוש הגובר בכפפות לטקס וגומי גרם לעלייה במספר מקרי האלרגיה ללטקס בין עובדי מערכת הבריאות. בשני סקרים שנערכו לאחרונה בישראל בבית חולים כללי ובבית חולים לילדים, נמצאה שכיחות נמוכה של אלרגיה ללטקס.

         

        מטרה: לבדוק שיעור אלרגיה ללטקס בין עובדי בית חולים כללי בישראל החשפוים למגע עם כפפות לטקס.

         

        שיטות: נכללו במחקר 203  עובדים של מרכז רפואי רמב"ם, ממחלקות שבהן יש שימוש רב בכפפות לטקס. כל עובד ענה בשאלון על פרטים דמוגרפיים, פרטים הקשורים למחלקתו ולפקידו, אנאמנזה אישית ומשפחתית על אלרגיות ופרטים קליניים הקשורים לחשיפה לכפפות לטקס. דגימות-דם ניטלו מכל הנכללים במחקר למטרת בדיקה כמותית של נוגדן IgE כללי ושל נוגדן IgE סגולי ללטקס, שנבדק בשיטת DPC Alastat-ELISA.

         

        תוצאות: 75 מבין 203 העובדים (36.9%) הצהירו על תופעות קליניות העלולות להיות קשורות לחשיפה ללטקס. רק 32 עובדים (15.8%) הצביעו על תופעות קליניות שניתן ליחסן לתגובת רגישות-יתר מיידית (אלרגיה מטיפוס I), כגון נזלת, התעטשויות, דמעת והפרעות-נשימה, בקרב 30 מהם (המהווים 14.8% מכלל העובדים) נמצאו נוגדני IgE סגוליים ללטקס (מעל 0.35 יב"ל/מ"ל), וב-21 מהם (המהווים 10.3% מכלל העובדים) נמצאו רמות גבוהות מאוד של IgE סגולי ללטקס (1.5 יב"ל/מ"ל). כמחצית מהעובדים (43.4%) עם נוגדני IgE סגוליים ללטקס היו רופאים והשאר אחיות (23.3%), עובדי משק (20.0%) ועובדי מעבדה (13.3%). המאפיינים המשמעותיים ביותר בקרב 30 עובדים עם אלרגיה ללטקס מטיפוץ I היו אנאמנזה אישית ומשפחתית של אלרגיה, רמות גבוהות של IgE כללי וחשיפה ללטקס מעל שעתיים ביום.

         

        מסקנות: היארעות תגובת יתר של רגישות מיידית ללטקס בקרב עובדי בית חולים כללי בישראל (14.8%) דומה לזו שנמצאה במרכזים רפואיים אחרים בעולם. עובדים עם אנאמנזה אישית או משפחתית של אלרגיה, החשופים ללטקס מעל שעתיים ביום ושבדמם נמצאה רמה גבוהה של IgEכללי, היו בסיכון גבוה ללקות בתופעות קליניות של אלרגיה ללטקס מטיפוס I. על סמך נתונים אלה, ניתן לאתר עובדים במערכת הבריאות שיש למנוע את המשך חשיפתם ללטקס, על-מנת למנוע או להפחית התהוות תופעות קליניות הקשורות באלרגיה ללטקס.

        Streptococus pyogenes ידוע כגורם לזיהומים, רובם מוגלתיים, בילדים ובמבוגרים כאחד. הזיהומים כוללים דלקת בלוע סעפת (impetigo), צלוליטיס, שנית (scariet fever), הלם אילחי (septic shock) תיסמונת ההלם האילחי (septic shock syndrome) ועוד. לחשיבותם הקלינית של הזיהומים מספר סיבות: 1) עד 20% מבני-האדם באוכלוסייה הם נשאים אי-תסמיניים של החיידק; 2) לעתים קרובות, התהליך הקליני של הזיהומים קשה ואף קטלני; 4) עלייה, הן בשיעור הזיהום והן בפולשניות החיידק, המתרחשת בכל העולם. צפקת ראשונית חדה הנגרמת על-ידי S. pyogenes, מהווה ביטוי קליני חריג. על מנת להגביר את ערנות הרופאים, מובאת בזאת פרשת חולה של תינוקת, שבה אובחנה צפקת ראשונית שנגרמה על-ידי S. pyogenes. האיבחון המוקדם והטיפול ששילב ניתוח דחוף ובחירה נכונה של טיפול אנטיביוטי, איפשרו החלמת החולה.

        Corneal Infection - Causes and Effects on Vision

        We conducted a retrospective 5-year survey of corneal infections treated in the ophthalmology ward of Hasharon Hospital. The most frequent type of corneal infection was corneal abscess; the most frequent cause was Staphylococcus albus, although this bacterium is not reported as a frequent cause of corneal infections.

        There was improvement in visual acuity in 69.2% and no change in 15.4%. Corneal infection by Pseudomonas aeruginosa was the most frequent cause of worsening of corneal acuity (23.08%). The greatest improvement of visual acuity was in those with corneal ulcers. The worst visual acuity was in those with corneal abscesses. Pseudomonas aeruginosa was the main cause of infection in contact-lens wearers.

        In the world medical literature, Staphylococcus albus is considered of very low virulence. This bacterium was the most frequent cause of corneal infections in our study so it may have greater virulence in Israel.

        Low Prevalence of Hepatitis G Infection in Dialysis Patients

        Prevalence of hepatitis G virus (HGV) infection in the general western population ranges from 0.2-1.5%. In high-risk groups, such as patients with chronic liver disease, hematologic disorders and drug addicts, prevalence is as high as 10%-15%. Dialysis patients have increased rates of HGV infection (6%-50%).

         

        We evaluated prevalence of HGV infection among dialysis patients, and the association between HGV infection and hepatitis C virus (HCV) infection. Serum samples were screened for HGV infection by RT-PCR. Screening for HCV infection was performed by an EIA test and confirmed by RIBA and RT-PCR for HCV. Sera were also tested for HBV markers.

         

        The study group included all 78 hemodialysis patients and 7 of the 12 peritoneal dialysis patients in our unit during September to November 1997. 4 (5.2%) were HGV-positive but none were peritoneal dialysis patients. 1 of the 12 HCV-positives was also infected with HGV. HGV infection was not associated with duration of dialysis, number of blood transfusions or levels of transaminases.

         

        Prevalence of HGV infection among our hemodialysis patients was low (5.2%), but higher than reported for the general population. Prevalence of HGV/HCV infection in hemodialysis patients was low and unrelated to duration of dialysis, number of blood transfusions and levels of transaminases.

        Listeria Monocytogenes Infections - Ten Years' Experience

        7 cases of listeriosis were diagnosed here between 1988-1997 (6 in last 3 years), or 2.94/100,000 admissions. 2 elderly patients suffered from meningitis and 2 pregnant women presented with premature contractions, 1 of whom delivered a premature, infected baby. 2 other patients had fever and gastroenteritis.

        Listeria monocytogenes was isolated from blood in 4, CSF in 2 and the placenta in 1. It was isolated from those with bacterial meningitis. All patients recovered. Both increased awareness for prevention and better diagnosis are essential to reduce morbidity from this unusual pathogen.

        Control of Brucellosis in Taibe: Intersectoral Collaboration

        Brucellosis is contracted from domestic animals. Poor hygiene, primitive animal breeding methods and traditional food preparation are the main contributory factors. We describe an intersectoral program for controlling brucellosis in Taibe, an Arab town in Israel, which had a particularly high incidence of the disease in 1992 and 1993.

        At the beginning of 1994 the Israel Ministry of Health and the Community Pediatric Center of Taibe established a community-based program for controlling brucellosis in Taibe. It included an intensive public health education campaign and periodic examination and vaccination of animals. Physicians, veterinarians, nurses, school officials and health inspectors were recruited for this purpose. Residents' awareness of brucellosis was determined before and after the study.

        After intervention, the incidence of the disease sharply declined from 176.6 and 175.0/100,000 in 1992 and 1993 respectively, to 5.7, 10.4 and 2.5/100,000 in 1994, 1995 and 1996, respectively, (odds-ratio 24.44; p<00000). Residents' awareness of brucellosis and preventive measures were significantly increased by the end of the study. We conclude that intersectoral collaboration is an important tool for controlling brucellosis.

        Infective Endocarditis Presenting as Polyarticular Septic Arthritis

        We report acute polyarthritis in an 86-year-old man which was initially considered to be an exacerbation of osteoarthritis. After a delay of a few days a diagnosis of septic polyarthritis as an initial symptom of infective endocarditis was made. Although rheumatic manifestations in infective endocarditis are common, septic polyarthritis is rarely seen. In our patient the course of the disease was rapidly fatal.

        Many Faces of Parvovirus

        2 women, aged 31 and 37 years, respectively, suffered from acute febrile illness due to acute infection with human parvovirus B19. 1 also had a maculopapular rash and articular symptoms after her fever stopped, a clinical picture typical of erythema infections. The other had leukopenia and thrombocytopenia and had received dipyrone. Although acute infection with human parvovirus usually occurs in childhood, it may also occur in adults, with protean manifestations.

        Neurological Manifestations of Non A-G Viral Hepatitis

        Guillain-Barre and other neurologic syndromes rarely occur as complications of viral hepatitis (A, B and C). Other neurologic syndromes have also been reported in serologically defined viral hepatitis, including mononeuritis, auditory neuritis, and seizures. Chronic hepatitis B and mononeuritis multiplex are found together in 31-54% of patients with periarteritis nodosa. The mechanisms of these associations are unknown, but may include direct cytotoxicity of the virus or immune-mediated damage. Vasculitis of the vasa nervorum plays an intermediate role, at least in some cases. We describe a 36-year-old man with acute non A-G hepatitis complicated by Guillain-Barre syndrome. The neurological manifestation resolved completely without specific therapy within 6 days, as the hepatitis resolved.

        Is Official Data on Reported Morbidity Valid? Hepatitis A in Israel as an Example

        Hepatitis A is one of the most frequently reported notifiable infectious diseases in Israel. The annual incidence as reported is around 70/100,000. The physician or the diagnostic laboratory notifies the district health office of the Ministry of Health.

        The purpose of this research was to evaluate the sensitivity of passive surveillance of hepatitis A morbidity among adults, 18 years and over. Methods included study of notifications to the Ministry of Health or hospitalizations of cases of hepatitis A and of positive laboratory tests results (IgM) for hepatitis A. We estimated the extent of under-reporting by 2 different methods of extrapolation.

        Data based on passive surveillance among the adult population, between 1.1.1993-31.12.1994, comprised less than 1/5 of the actual number of cases. Physicians notified about 6.2% of their hepatitis A patients. 5.1% of the notifications to the district health office were sent twice or more, usually both by the physicians and labs.

        The official data on hepatitis A morbidity, based on passive surveillance, are considerably underestimated. Physicians and public health officials should be aware that such data may not accurately reflect the magnitude of the risk or the amount of disease that can be prevented. Efforts should be made to improve this situation.

        Brucellosis Presenting as Acute Abdomen

        Usually symptoms of brucellosis are nonspecific and characterized by a wide range of complaints. Although the disease in Israel is almost exclusively food borne (caused by Brucella melitensis in unpasteurized goat milk products) so the main route of infection is the gastrointestinal tract, but gastrointestinal complications are rare, and only sporadic cases of ileitis or colitis have been described.

        We present a 43-year-old woman with an acute abdomen, probably due to diverticulitis. It was diagnosed only after blood cultures were positive for Brucella melitensis. We believe that its protean manifestations should be consin addition to the other bizarre presentations of this disease, important in our region.

        Meningitis Due to Streptococcus Bovis Type 2

        Meningitis due to Streptococcus bovis is rare. Only 14 cases having been reported in the English literature. All patients (including the patient described) had an underlying disease or were treated by pharmacological agents that predisposed the patient to the infection. Most were treated by monotherapy with penicillin G (or amoxicillin) and recovered.

        We describe a 74-year-old woman who had splenectomy as treatment for hairy cell leukemia 6 months before hospitalization for meningitis and sepsis by S. bovis type 2. She was successfully treated with intravenous amoxicillin. There was neither evidence of endocarditis nor carcinoma of the colon. Although the association between S. bovis meningitis and endocarditis or carcinoma of the gastrointestinal tract is not well established, we recommend a full work-up for GI malignancy and endocarditis in every patient with S. bovis meningitis.

        Epidural Spinal Abscess

        A 42-year-old man was admitted for fever and severe low back pain radiating to both legs. On MRI, an epidural spinal abscess from S1 to D10 was seen. Treatment included laminectomy, drainage of the abscess and antibiotics. Recovery was complete without neurological damage. Increased awareness of this disease may lead to diagnosis and treatment.

        Nasopharyngeal Colonization with Streptococcus Pneumoniae in Pediatric Respiratory Infections

        During the winter of 1995, nasopharyngeal colonization of Streptococcus pneumoniae was evaluated in 204 children with respiratory infection and 107 normal control children. There was no difference in gender or mean age between the groups, and no difference in carrier rate between sick (24.5%) and normal (22%) children (p=0.6). Carrier rates were 19%, 32%, 31% and 17% at 6, 12, 24 and 48 months, respectively. Penicillin-resistant pneumococci (PRP) were found in 42% of sick and 16.6% of normal children, (p<0.05). Resistance to more than 2 antibiotics was found in 28% of sick and in 12.5% of normal children. PRP were found in 67% and 34% of sick children with and without prior antibiotic treatment (p<0.05). We conclude that there is no difference in the carrier rate of Streptococcus pneumoniae between sick and normal children. However, the high prevalence of PRP in children with respiratory infections is probably due to prior antibiotic treatment.

        The Diagnosis and Treatment of Fournier's Gangrene

        We treated 2 women and 8 men suffering from Fournier's gangrene during 1990-96. 2 had diabetes, 1 suffered from ulcerative colitis and 1 was an alcoholic. In 8 of them the infection was triggered by a mixture of aerobic and anaerobic bacteria. Treatment consisted of repeated wide debridement and early colostomy. This aggressive approach resulted in relief of the septic signs within 24 hours and permitted early skin grafting of the wounds. 2 patients died due to sepsis that caused multiple organ failure. The 8 who survived were hospitalized for an average of 35 days. On follow-up examination 1-5 years later all patients had undergone closure of the colostomy and were completely rehabilitated.

        Fournier's gangrene is not rare in the geriatric population. We believe that early diagnosis and aggressive wide debridement, combined with early colostomy, are the keys to successful treatment.
