Background: Increased utilization of imaging modalities has led to a significant rise in the detection of incidental thyroid nodules (ITN). Discrepancies in the prevalence of thyroid nodules with malignant potential exist worldwide.

Objectives: To analyze demographic and clinical data among patients with thyroid nodules in our geographic region.

Methods: The medical records of patients diagnosed with symptomatic or incidental thyroid nodules at the Galilee Medical Center between 2018 and 2023 were reviewed. Demographic and clinical data were collected and analyzed.

Results: The study population included 402 patients with thyroid nodules, 292 females. Symptomatic patients were younger (mean age 55.9 vs. 60.8 years) and had larger nodules (mean size 2.5 vs. 2.1 cm) compared to incidentally diagnosed patients (P < 0.001, P < 0.001, respectively). Male patients demonstrated a higher rate of malignancy for both symptomatic and incidental nodules compared to females (P < 0.05). Pathological examination revealed that malignant nodules were smaller (mean size 2.10 cm vs. 2.87 cm) and detected at a younger age (mean age 48.56 years vs. 56.5 years), compared to benign nodules (P < 0.05, P < 0.01, respectively).

Conclusions: We found a higher prevalence of both symptomatic and ITN among females. However, malignant thyroid nodules were more frequently observed in males. Notably, malignant nodules tended to be smaller and were more commonly diagnosed in younger individuals compared to benign nodules. These findings highlight significant sex and age disparities in the occurrence and characteristics of thyroid nodules, emphasizing the need for tailored diagnostic and management strategies.

Background: The 2015 American Thyroid Association (ATA2015) and the American College of Radiology Thyroid Imaging and Reporting Data System (ACR TI-RADS) are two widely used thyroid sonographic systems.

Objectives: To compare the two systems for accuracy of cancer risk prediction.

Methods: Preoperative ultrasound images from 265 patients who underwent thyroidectomy at our hospital from January 2012 to March 2019 were retrospectively categorized by the ACR TI-RADS and ATA2015 systems. Diagnostic performances were compared.

Results: Of 238 nodules assessed, 115 were malignant. Malignancy risks for the five ACR TI-RADS categories were 0%, 7.5%, 11.4%, 59.6%, and 90.0%. Malignancy risks for the five ATA2015 categories were 0%, 6.8%, 17.0%, 55.5%, and 92.1%. The proportion of total nodules biopsied was higher with the ATA2015 system than the ACR TI-RADS system: 88.7% vs. 66.3%. Proportions of malignant nodules and benign nodules biopsied were higher with ATA2015 than with ACR TI-RADS: 93.3% vs. 87.8% and 84.4% vs. 46.3%, respectively. Specificity and sensitivity rates were 53.6% and 84.3%, respectively, for ACR TI-RADS, and 15.5% and 93.3%, respectively, for ATA2015. The two systems showed similarly accurate diagnostic performance (AUC > 0.88). False negative rates for ACR TI-RADS and ATA2015 were 15.6% and 6.6%, respectively. Rates of missed aggressive cancer were similar for the two systems: 3.4% and 3.7%, respectively.

Conclusion: ACR TI-RADS was superior to ATA2015 in specificity and avoiding unnecessary biopsies. ATA2015 yielded better sensitivity and a lower false negative rate. Identification of aggressive cancers was identical in the two systems

Background: Fine needle aspiration is the main diagnostic tool used to assess thyroid nodules.

Objectives: To correlate FNA[1] cytology results with surgical pathological findings in two teaching medical centers across the Atlantic.

Methods: We retrospectively identified 484 patients at Hadassah University Hospital, Jerusalem and Mount Sinai Hospital, New York, by means of both preoperative FNA cytology and a final histopathological report. Results compared FNA diagnosis, histological findings and frozen section results (Mt. Sinai only).

Results: The sensitivity value of FNA at Hadassah was 83.0% compared with 79.1% at Mt. Sinai (NS). Specificity values were 86.6 vs. 98.5% (P < 0.05), negative predictive value 78.7 vs. 77.6% (NS) and positive predictive value 89.7 vs. 98.6% (P < 0.05), respectively. "Follicular lesion" was diagnosed on FNA in 33.1% of the patients at Hadassah and in 21.5% at Mt Sinai (P < 0.005) with a malignancy rate of 42.5 vs. 23.1% (P < 0.05), respectively. Frozen section was used in 190 patients at Mt. Sinai (78.5%) with sensitivity and specificity values of 72.3% and 100%. Frozen section results altered the planned operative course in only 6 patients (2.5%). Follicular carcinoma was diagnosed in 12 patients at Hadassah vs. 2 patients at Mt. Sinai (P p <0.05).

Conclusion: The sensitivity of FNA at the two institutions was comparable. While malignancy on frozen section is highly specific, it should be used selectively for suspicious FNA results. Follicular lesions and the rate of malignancy in such lesions were more common at Hadassah, favoring a more aggressive surgical approach.
